We are located in a four building business complex just east of State Street and Zion's Bank. The large red brick buildings of "Independence Square" are easy to see with the western building's easily visible tall white clock tower. The LifeStar Salt Lake Clinic is on the 3rd Floor of the northern "Washington Building."

Directions Coming from Interstate 15
Traveling on I-15 (the North/South freeway traversing the Salt Lake valley), find the 5300 South exit (Exit #300) and proceed East (towards the large IMC Hospital and Costco). Turn right (South) on State Street and the second lighted intersection is 5600 South. Make a left turn (East) and beyond the Zion's Bank you'll see our colonial style business complex called Independence Square.
We are in the northernmost building (the "Washington Building") marked #151 on the white pillar. Our offices encompass the 3rd floor, but proceed to Suite 308 for further information.
Accessiblity
Wheelchair/freight access is on the Northwest corner of our building. The elevator is centrally located in the building.
